I. Minor and Double Major Applications
1. Application period: Tuesday, September 29, 2026, to Monday, October 5, 2026, 5:00 p.m.
2. How to apply: Submit the required documents to the Department of Health and Welfare Office (Room C314, 3rd Floor, Administration Building, Tianmu Campus) by the deadline.
3. Eligibility:
(1) Minor:
a. Undergraduate students may apply for a minor from the second semester of their first year through the first semester of their fourth year (i.e., students currently in this range who are interested may apply).
b. Please attach the application form, an official transcript (including class ranking), and documentation of club participation or other outstanding achievements.

(2) Double Major:
a. Undergraduate students may apply for a double major from their second year through the first semester of the final year of their program (i.e., students currently in the second semester of their first year through the second semester of their third year who are interested may apply).
b. Please attach the application form, an official transcript (including class ranking), and documentation of club participation or other outstanding achievements.

4. Screening: The date and time will be announced separately. Applicants must bring their student ID card to check in.
5. The final list of admitted students will be announced by the Office of Academic Affairs.

II. Health Care Social Work Credit Program Applications
1. Application period: Tuesday, September 29, 2026, to Monday, October 5, 2026, 5:00 p.m.

III. English-Taught Credit Program in Sports and Health Applications
